Medically complex children average 13 practitioners and 6 specialists- they have a lot going on!
Brades' Place coordination allows all providers, specialists, & other professionals to work together seamlessly, fully aware of all other ongoing work, augmenting and supporting each other's care, and connecting the many moving parts of treatment.
Often coordinating complex medical needs falls to the parents. As we take this lead, parents are enabled to put their energy on their family and their child.
We also facilitate health care team conversations about disease trajectory, treatment options, comfort & pain needs, unmet medical needs, and patient and family wishes as treatment ends and disease advances.
Frequent review of family values & goals ensures that they are supported by the current medical treatment plan.
